Manifest MedEx gives providers a high-resolution view of every patient’s journey by securely bringing together clinical records, claims data, behavioral health, and social care data. Providers can instantly access comprehensive patient histories, lab results, and medication lists through MX Access at the moments that matter most.

Automated hospital event notifications through MX Notify alert providers in real time so they can follow up with patients faster after a hospital visit, helping reduce avoidable readmissions and ensure patients get timely follow-up care.

Providers can see social care information that is captured within clinical records already flowing through the MX network, such as Z-codes and referral information documented as part of a person’s care. This is part of MX’s core HIE data, available to providers by default. Full access to MX’s dedicated Social Health Information Exchange (branded MX Engage), which includes a broader range of social drivers of health data, care assessments, and closed-loop referral tools, requires a separate subscription.

As a designated Qualified Health Information Organization (QHIO), Manifest MedEx helps providers meet California Data Exchange Framework (DxF) data-sharing requirements without needing to build their own infrastructure or respond individually to requests from thousands of other DxF signatories.

No. Primary care providers generally do not have to pay fees to join Manifest MedEx or to connect through their own EHR vendor. Manifest MedEx already connects with more than 70 different EHR systems across California, so many providers can connect without needing to build a custom data pipeline.

Ambulatory providers make up one of the largest groups in the Manifest MedEx network, with more than 3,200 participating practices across California. MX gives ambulatory providers access to real-time ADT notifications, comprehensive longitudinal patient records, and social care information, all at no cost, helping independent and small practices access the same level of care coordination tools as larger health systems.

Yes. Through participation in national networks like eHealth Exchange and Carequality, and connection to the Trusted Exchange Framework and Common Agreement™ (TEFCA®), Manifest MedEx can exchange permitted health data with providers and health systems outside of California, helping ensure continuity of care for people who receive treatment in multiple states.
