Partner with MX

Manifest MedEx supports CBOs and other health-related social services (HRSS) organizations to connect and exchange health and social services information, helping them support people experiencing health-related social needs and advance California’s CalAIM initiative.

By joining the MX network, CBOs can receive real-time admission, discharge, and transfer (ADT) notifications alerting them when a person they serve is in the hospital, in order to help arrange support such as housing, transportation, or medically tailored meals before or shortly after discharge. Notifications can be delivered through the MX web portal or as CSV files, and CBOs also receive a contact report to help their teams track and follow up on these notifications.

Access depends on the CBO’s status as a HIPAA covered entity. CBOs that are HIPAA covered entities can access protected health information (PHI) through MX Access, similar to other clinical participants in the network. CBOs that are not covered entities receive HRSS (health-related social services) access, a more limited view of MX Access designed to show only what’s needed to support their care coordination role. In both cases, this access is to data within MX’s core HIE (MX Access), separate from MX’s dedicated Social Health Information Exchange (MX Engage), which offers a broader range of social care data and tools through a separate subscription.

MX Engage is Manifest MedEx’s social health information exchange (SHIE) platform. CBOs that subscribe to MX Engage can use it to coordinate social services, including closed-loop referrals, alongside the clinical and claims data already available through the MX network, helping ensure that when someone is connected to a service like housing or transportation, that connection is tracked through to completion.

Manifest MedEx helps CBOs participating in Enhanced Care Management (ECM) identify and support eligible individuals by providing real-time ADT notifications and access to relevant clinical data, helping care teams coordinate outreach and services more effectively for people with complex health and social needs.
