Real-World Impact

California Roots. National Connectivity.

Ready to explore what’s possible with the state’s largest non-profit health data network?

Manifest MedEx provides hospitals with real-time admission, discharge, and transfer (ADT) notifications through MX Notify, access to comprehensive longitudinal health records through MX Access, and support meeting regulatory requirements, all designed to reduce manual faxes, streamline ED throughput, and improve care coordination.

Hospitals can meet California Data Exchange Framework (DxF) data-sharing requirements by selecting Manifest MedEx as their QHIO to support all required transaction patterns, or by participating in the California ADT Network, a no-cost service that enables the secure, bi-directional exchange of basic ADT alerts between DxF participants.

Since May 2021, CMS has required hospitals to send admission, discharge, and transfer (ADT) event notifications as part of the Conditions of Participation (CoPs) to improve care coordination. By joining the MX network and setting up a single data feed, hospitals can meet these CMS requirements quickly and with minimal cost.

Through MX Notify’s real-time ADT notifications, care teams and providers are immediately alerted when a person they are caring for is admitted, discharged, or transferred in a hospital setting, allowing timely follow-up and coordinated post-discharge care that helps reduce avoidable readmissions.

MX Access gives ED clinicians instant access to a patient’s comprehensive longitudinal health records, including medical history, medications, and lab results, helping inform clinical decision-making the moment a patient arrives.

Yes. Manifest MedEx helps hospitals reduce manual faxes and time-consuming chart requests by automating data delivery to health plans and providers and by ensuring discharge summaries reach a person’s PCP and care team immediately.

Beyond meeting CMS and DxF requirements, hospitals save meaningful time with MX automating the delivery of post-discharge summaries and sharing NCQA validated clinical data with health plans on their behalf, reducing the number of individual chart requests hospitals would otherwise need to respond to for new members, ongoing care coordination, and to help with utilization management reviews. Hospitals also gain access to patient information from systems not already connected to their own EHR for a more complete view of a patient’s health history in the community.
