Since May 2021, CMS has required all hospitals to send Admission, Discharge, and Transfer (ADT) Event Notifications as part of the Conditions of Participation (CoPs) to improve care coordination. These simple but powerful alerts notify a care team or provider when a patient has been admitted, discharged, or transferred from a hospital, setting into motion a series of events and processes that can streamline care coordination, reduce costly readmissions, and most importantly, improve quality and outcomes for patients.
In this webinar, guest speakers share how their healthcare organizations and patients have benefited from ADT notifications and longitudinal patient records that integrate clinical data, social drivers of health (SDOH), and other demographic information.

Data Sharing Agreement (DSA): Stakeholders Weigh In on Pivotal Policies
On March 21, Manifest MedEx joined other key voices in Sacramento to advise and advocate on crucial DSA decisions that are set to be finalized by the CalHHS Center for Data Insights and Innovation (CDII). In a joint meeting of its stakeholder advisory committees, CDII outlined its proposed responses to public comments on draft DSA policies-the most critical being whether to require hospitals to send ADT event notifications to at least one Qualified Health Information Organization (QHIO).
Manifest MedEx has channeled many of our participants in our strong support for keeping this ADT requirement to advance digital equity and provide a health data safety net that all DSA signatories can rely on to receive the ADTs they need for their patients, members, and clients. A diverse array of stakeholders-including physician groups, health plans, and care team partners in CalAIM and practice transformation-also called into the meeting to express their collective need for ADTs to flow through a robust and streamlined source that only QHIOs can provide.
Manifest MedEx also leveraged our technical and policy expertise to recommend immediate real-time standards for sending ADTs and no more than an hour for query responses, and to caution against overly broad restrictions on the scope of queries and information delivery.
Lastly, CDII updated stakeholders on the upcoming DSA Signatory Grants, which should be released in the second quarter of 2023 to allow organizations required to sign the DSA to begin applying for financial support with implementing data exchange, including QHIO onboarding. CDII newly proposed allowing “umbrella organizations” such as IPAs and MSOs to apply for these grants on behalf of multiple DSA signatories. CDII is now targeting April to release the QHIO Program application and June to announce its QHIO designation decisions.

Manifest MedEx Selected to Participate in DHCS’ Authorization to Share Confidential Medi-Cal Information (ASCMI) Pilot
Manifest MedEx is pleased to announce that in collaboration with San Joaquin County, Health Plan of San Joaquin, and Health Net, we are one of three selected participants in the California Department of Healthcare Services (DHCS) Authorization to Share Confidential Medi-Cal Information (ASCMI) Form and consent management service pilot (collectively referred to as the “ASCMI Pilot”).
The ASCMI form is a voluntary universal release of information form designed to facilitate sharing health information with all service providers through a standardized consent process. It allows an individual to authorize organizations to share their health information when consent is required, which may include substance use disorder (SUD) data and sharing health information with non-HIPAA covered entities.
“We are delighted to be selected by the state to pilot the ASCMI form and develop a consent management service,” said Erica Galvez, Chief Executive Officer, Manifest MedEx. “This is important work to ensure the success of CalAIM, which depends on seamless care coordination across physical health, behavioral health, and social service providers to support whole person care for Medi-Cal enrollees.”

MX Welcomes New Participant!
Manifest MedEx is excited to announce that we have partnered with the Center for Inherited Blood Disorders (CIBD) on a project targeted to serve sickle cell patients at several clinics throughout California. The organization was founded in 2009 by Dr. Diane Nugent, a world-renowned blood disorder expert and principal investigator for Region IX/Western States Hemophilia Network. By partnering with MX, Dr. Nugent and her dedicated care team gain access to patient data that will assist in providing the best care and outcomes for patients managing sickle cell. CIBD also has its own hemophilia treatment clinic in Orange, CA., where data from MX will be utilized for patient care for an array of blood disorders.
We are excited to work with CIBD to help them achieve their vision of “longer, healthier, and more productive lives.”
MX Health Equity and Social Drivers of Health (SDOH) Report
Improving health equity requires quicker insights and better data to understand population health, reduce health disparities, close care gaps, and improve access to proper care.
Manifest MedEx is excited to offer a new report, our Health Equity and SDOH Report, that provides health care organizations with the demographic and SDOH data they need to better understand their patient and member populations; identify and reduce health disparities; and prioritize populations and communities that are historically underserved to advance heath equity.
This report provides accurate individual-level data on race, ethnicity, and language, as well as SDOH data, such as Z-Codes and Public Health Alliance of Southern California’s Healthy Place Index, which incorporates economics, education, healthcare access, housing, neighborhood conditions, pollution/clean environment, social, transportation and racial residential integration domains. This timely data gives participants actionable insights into the needs of those they serve, including social risk factors and changes in communities’ needs.

ICYMI: eHealth Exchange, CSRI Announce QHIN Partnership Intentions
As a part of the Consortium for State and Regional Interoperability (CSRI), Manifest MedEx intends to partner with eHealth Exchange, one of the nation’s oldest and largest health information networks, on their anticipated Qualified Health Information Network (QHIN) to participate in the Office of the National Coordinator for Health IT’s (ONC’s) Trusted Exchange Framework and Common Agreement (TEFCA).
Founding CSRI health data networks pursuing the prospective eHealth Exchange QHIN include Contexture, CRISP, CyncHealth, and Indiana Health Information Exchange.
