Manifest MedEx and Orange County Partners in Health (OCPH)
Health Information Exchange (HIE) Enter Strategic Affiliation
To Advance Data Connectivity

Manifest MedEx has entered into a new strategic affiliation with Orange County Partners in Health (OCPH) Health Information Exchange (HIE), a DxF QHIO in Orange County that is a part of the nonprofit Coalition of Orange County Community Health Centers. With this partnership, OCPH-HIE participants will onboard to MX’s best-in-class technology to exchange data and support connectivity and coordination of care across Orange County.
“Manifest MedEx has records for more than 1.5 million Orange County residents, and 14 hospitals that serve our county are already on their network, sending data including ADTs and discharge summaries in real time to local providers,” said Isabel Becerra, President and CEO, OCPH-HIE. “It was important for us to find a partner with the expertise and depth of network to help our participants better serve their patients with access to their most recent data. Becoming an affiliate of Manifest MedEx was a natural choice for us.”
OCPH-HIE was established in 2020 as a project under the Coalition of Orange County Community Health Centers (Coalition OC), which represents the interests of 20 community health centers and one community-based organization that serve the community’s most at-risk residents. Since then, OCPH-HIE has expanded beyond the Coalition OC’s membership to include private practices, hospitals, and specialty clinics.
Under this affiliation, OCPH-HIE participants will continue to meet DxF data sharing requirements.

"Powering Healthy Communities Through
Whole Person Data Exchange" Recap

Manifest MedEx hosted half-day regional convenings on April 8 in Rocklin and on April 10 in Merced to advance opportunities to improve whole-person care coordination and address California regional priorities.
We kicked off both days with a presentation from MX's Director of Health Policy, Felix Su, who provided insight into how DxF oversight and enforcement by state agencies are poised to ramp up over the next several years, and the best opportunities to comply with and benefit from requirements for secure, real-time data sharing.

On April 8, attendees gained an on-the-ground perspective from Lisa Chan-Sawin, Chief Executive Officer and Founder of Transform Health, as she shared the status of CalAIM in Central and Northern CA, notable aspects of the 1115 renewal application, and best practices and opportunities for local health organizations to advance whole person data sharing.
After lunch, MX's Executive Director of the Central and Northern Regions, Ednann Naz, M.D., MPHS, MBA, moderated a discussion where Edward Kalpas, M.D., MAS, MPH, Chief Medical Officer, Peach Tree Healthcare; Samantha Parker, Manager, Service Delivery, Health Plan of San Joaquin; and Michael Selberis, Chief Technology Officer, ClinNEXUS, highlighted the evolution of health data exchange in their communities, its impact on their Medi-Cal populations through services, and opportunities to strengthen care coordination and support person-centered care.

On April 10, MX's Business Development team made their way to Merced where Fresno County Department of Public Health's Assistant Director, Claudia Sandoval, and Division manager of Epidemiology, Surveillance, & Data Management, Marlene Pena, kicked off the late morning session with how the Fresno County Department of Public Health approaches whole person care, while highlighting the newly launched Fresno County Information Exchange – a collaborative data infrastructure that supports holistic coordination of care through coordinated, connected health data exchange.
After lunch, Victoria Worthy, Chief Information Officer, Health Plan of San Joaquin, presented on how cross-sector collaboration is advancing whole person data sharing, supporting data-driven decision-making, and improving population health outcomes in the Central Valley and surrounding communities. Then, MX's Chief Business Development Officer, Adam Harrison, joined Victoria for a fireside chat to explore key insights, lessons learned, and opportunities to strengthen collaboration across the region.
Community-driven dialogue on Connecting the Dots: Partnering with HIOs to Meet Local Community Priorities
Both convenings ended with transparent dialogue on real-world use cases that demonstrate how a more connected health and social care data-sharing ecosystem advances innovation and equity across the local healthcare landscape.
We enjoyed connecting and collaborating with local and regional industry leaders to better understand critical gaps, review lessons learned, and curate a path forward as Manifest MedEx continues to enhance our products and services to meet regional needs and advance whole person care throughout the state of California.

Manifest MedEx welcomes Artesia Christian Home (ACH) to our ever-growing network!
Since 1951, ACH has aimed to provide a continuum of high-quality services to the faithful members of their supporting churches and to the community. Rooted in providing Christ-like, compassionate care through tangible acts of love, ACH is a 66-bed skilled nursing facility that includes 24 specialized beds for patients in the final stages of dementia. ACH is delighted to minister to the physical, emotional, and spiritual needs of those it serves, regardless of race, color, or national origin.
Manifest MedEx looks forward to providing hospital discharge summaries from their referring hospitals via MX Access to close care gaps and advance coordination among their members, while ensuring compliance with the Data Exchange Framework.

On May 14 from 12:00 p.m. - 1:00 p.m. PT, MX's Chief Information Officer, Jason Buckner, will join Wisconsin Statewide Health Information Network's Director of Population Health and Analytics, Matt Gigot, to discuss real-world lessons from earning NCQA Data Aggregator Validation (DAV) status and share practical tips for organizations that are planning to begin or revisit their validation effort in a webinar moderated by J2 Interactive's Director of Professional Services, Nathan Elliott.
