Manifest MedEx 2025 Annual Report
We are pleased to present Manifest MedEx’s 2025 Annual Report, highlighting our measurable progress and growing impact of a connected health data sharing ecosystem within California communities and across more than 50M individual records.
To reflect our organization’s growth and expansion over the past eight years, we spent thoughtful time last year updating our mission and 10-year vision. This was a deeply collaborative process — intended to both align with the value we’re delivering today and set a clear path forward for our next decade of work:

High-resolution data from Manifest MedEx optimizes health and value for all
In 2025, our participant network grew by 12% as we added ambulatory, supplemental services, and health-related social services providers; new organizations from county and state agencies; skilled nursing facilities; and more health plans, hospitals, and health systems. Our annual delivery of ADT notifications increased by 35%, helping more care teams use these critical alerts to coordinate care when it matters most.
Our partnerships with community care network platforms, California Mental Health Services Authority (CalMSHA), and Serving Communities Health Information Organization (SCHIO) are helping us expand the delivery of whole person health data, and we invested in ways to improve data quality to provide more accurate and actionable data.
We hope this report shows how MX is not only building health data infrastructure but also providing more complete, timely, and accurate data needed to support our participants, extend value beyond our network, and help advance California’s broader health goals.

MX Welcomes New Participants
Manifest MedEx welcomes the San Bernardino County Department of Public Health and Department of Behavioral Health, Mariposa County Health and Human Services Agency, McKinley Children’s Center, and the Monterey County Department of Public Health and Department of Behavioral Health to our ever-growing network!
The San Bernardino County Department of Public Health (DPH) aims to promote, improve, and protect health, wellness, safety, and quality of life in San Bernardino County, while the San Bernardino County Department of Behavioral Health (DBH) strives to be recognized as a progressive system of seamless, accessible, and effective services that promote prevention, intervention, recovery, and resiliency for individuals, families, and communities. Manifest MedEx is pleased to provide both departments access to timely, actionable, longitudinal health data through MX Access to support DPH in timely intervention and the identification of opportunities for new data-informed public health programs, while facilitating connections between DBH and individuals who have behavioral health diagnoses at San Bernardino County emergency departments, ensuring they receive appropriate services.The Mariposa County Health and Human Services Agency (HHSA) empowers individuals, families, and the community to be healthy and safe by providing integrated public health, behavioral health, and social services to residents of Mariposa County. Through responsive, caring, and supportive services, HHSA works to improve the health and well-being of the community through prevention, care coordination, and partnerships with healthcare providers and community organizations. By partnering with MX, Mariposa County will gain access to longitudinal health records, encounter notifications, and reporting tools that support care coordination and population health insights.
Since their founding, McKinley Children’s Center has supported children who are survivors of trauma—many from homes impacted by abuse, neglect, or instability. From the beginning, they’ve provided safe housing, education, and care to help each child feel valued and capable of healing. What started as a home for boys has grown into a far-reaching network of services. Today, McKinley continues to serve some of Southern California’s most vulnerable youth and families with compassion and consistency. MX looks forward to providing real-time notifications to McKinley Children’s Center to help them provide the best mental health, educational, and residential services for their communities.
The Monterey County Department of Public Health oversees communicable disease prevention, public health nursing, and the WIC program, while the Monterey County Department of Behavioral Health promotes wellness, resilience, and recovery through prevention and partnering to provide welcoming mental health and substance use care for over 12,000 individuals, encompassing adults, youth, young adults, children, families, and caregivers. Manifest MedEx looks forward to partnering with both Monterey County departments and providing access to clinical records, claims information, labs, discharge notes, and real-time hospital notifications.
