MX's Inaugural Regional Forum

On September 9th, we hosted our inaugural regional forum, Evolution CA 2025: Southern Inland, themed “Bridging Clinical, Claims, and Social Data to Accelerate Whole Community Care,” at the March Field Air Museum in Riverside, California.
Key stakeholders from the local, regional, and state levels – including hospitals, health plans, social support organizations, and health agencies – convened for a collaborative discussion on advancing health and health-related social services (HRSS) data exchange to support healthy communities.

MX Product Roadmap Quarterly Update

Manifest MedEx continues to enhance our products and services to improve workflow, ensure data security, and save network participants' time when coordinating essential care for their patients, members, and clients. Here are a few glimpses into what MX participants can expect in Q4 of 2025:

Release of County Population Health Reports – MX has developed a series of eight population health reports to assist counties in tracking the health of their county residents by zip code and census tract. These reports, which can be produced and delivered quarterly, provide insight into the following important health domains and attributes: behavioral health, chronic conditions, immunizations, preventative care, sexual health, social drivers of health, substance use disorders, and healthcare utilization. Intended to be used in conjunction with counties’ existing population health platforms and tools, these reports contain important information to assist with population stratification, including geography, age groups, gender and gender identity, sexual orientation, race, ethnicity, and language. MX’s County Population Health Reports are available beginning October 1st, 2025.

MX Enhanced NCQA Data Aggregator Validation Services and MX Enhanced Longitudinal Patient Summaries in CCDAs Service
– To help health plans and risk-bearing entities report more accurately and completely on HEDIS® measures, close care gaps, and improve quality, Manifest MedEx is offering enhanced, normalized data in our NCQA Data Aggregator Validation Services and Longitudinal Patient Summary service. Health organizations often struggle with inconsistent coding, local variations, and missing structure in data from clinical settings. Health information that is not properly coded can result in many challenges, including difficulty determining the specifics of what care has been provided and identifying individuals with certain conditions. Manifest MedEx improves the quality and usability of member data by mapping local codes to standard codes. We are pleased to offer this capability for validated data for HEDIS® reporting, and with our longitudinal patient summary service!

MX Welcomes New Participants

Manifest MedEx welcomes Astiva Health PlanSenior Care Action Network (SCAN), Southern Humboldt Community Healthcare District (SoHum Health),  and TriValley Medical Group to our ever-growing network.

Serving over 30,000 members across California, Astiva Health is a Medicare Advantage plan that aims to help its members live life through tailored, comprehensive member packages that aim to meet members' personalized needs and best support their local, underserved member population. MX will help Astiva Health improve post-discharge care coordination and member engagement by providing real-time admission, discharge, and transfer (ADT) notifications.

As one of the largest not-for-profit Medicare Advantage plans in the nation, launching its first Medicare health plan in 1984, Senior Care Action Network (SCAN) was established in 1977 and dares to care differently. SCAN serves over 300,000 members across Arizona, California, Nevada, New Mexico, and Texas, and is a driving force for healthy and independent aging in America. MX will partner with SCAN to help them gain access to real-time ADT notifications that will aid in the facilitation of timely engagement with SCAN members following a hospital visit.

Southern Humboldt Community Healthcare District (SoHum Health) was established in 1980 to empower individuals to live longer, healthier lives. With core values of care, quality, teamwork, and positivity, SoHum Health is committed to providing accessible, high-quality healthcare to the residents of Southern Humboldt County. By offering access to longitudinal patient records, real-time ADT alerts, and statewide interoperability, MX looks forward to helping SoHum Health gain timely insight into care transitions, improve collaboration with regional providers, and help support clinical and quality initiatives.

Providing the highest quality, technologically advanced, comprehensive medical care since 2002, TriValley Medical Group is the first hospitalist group to build a practice, IPA, and now a health system, with a foundational approach that aspires to have national impact and create better outcomes and better care. MX will support better patient care outcomes and help TriValley Medical Group reduce emergency (ED) and inpatient (IP) readmissions by utilizing real-time ADT notifications. This partnership will also help create a closed-loop system aligned with TriValley's environmental, social, and governance (ESG) model.

MX Renews HITRUST Certification

Manifest MedEx is pleased to announce that we have earned HITRUST Certification for the seventh consecutive year! 2025 Civitas Networks for Health® Annual Conference

Mark your calendars for the 2025 Civitas Networks for Health® Annual Conference!

On September 29, MX’s Jason Buckner, IEHP’s Genia Fick, and NCQA’s Wendy Talbot, MPH, discussed how collaborative efforts across payers, providers, and health data organizations have strengthened data accuracy, completeness, and usability—leading to more reliable reporting, better care coordination, and improved patient outcomes.
