Evolution CA 2026: Bay Area

Manifest MedEx is hosting a one-day regional forum focused on collaboration, innovation, and creative problem-solving as we bring together stakeholders from the Bay Area and nearby communities to discuss how whole person data exchange can support healthier communities and ensure people receive timely care informed by all aspects of their health.

Hear from state, regional, and local experts as they provide real-world use cases that highlight opportunities for collaboration between health care, behavioral health, and social care organizations to support whole person care. Attendees will gain insight into the latest policies impacting health and social health information exchange, how local and state agencies are supporting county-wide data exchange ecosystems to advance population health, how to optimize Enhanced Care Management and Community Supports through better care coordination, and innovations in ways to connect and power healthier communities.

If you are interested in attending our regional forum, Evolution CA 2026: Bay Area, please contact us at info@manifestmedex.org.

MX Welcomes New Board Member

Manifest MedEx is committed to optimizing clinical, behavioral, and social care information exchange to build healthier communities across CA while advancing whole person care, closing care coordination gaps, and improving outcomes. To support these goals, MX welcomes Ikenna (Ike) Mmeje, President & CEO, USC Arcadia Hospital, to our Board of Directors.

Mmeje furthers Keck Medicine’s mission to expand access to specialized health care and research to the San Gabriel Valley and beyond. He oversees all management and operations of the hospital, including corporate compliance, strategic plan implementation, board development, and fundraising.

MX Welcomes New Participants

Manifest MedEx welcomes Amador County Behavioral Health Department, Children's Hospital of Orange County, and Hazel Hawkins Memorial Hospital to our ever-growing network!

Amador County Behavioral Health Department provides mental health and substance use services to residents, with a focus on improving access, coordination, and outcomes for vulnerable populations. In the absence of real-time data, the County has limited insight into the social and clinical drivers of health, resulting in fragmented care coordination. MX looks forward to providing real-time alerts, longitudinal records, and advanced reporting tools to improve visibility into patient care across settings, enhance coordination with hospitals and providers, and support population health and equity initiatives.

What started as a 62-bed children’s hospital – the first in Orange County – has grown into a pediatric health care system that includes two state-of-the-art hospitals in Orange and Mission Viejo and a regional network of primary and specialty care clinics serving children and families in four counties. Since 1964, the Children's Hospital of Orange County (CHOC) has advanced the hope, health, and healing of children through innovative care and is committed to being a leading destination for children’s health by providing exceptional, innovative care. MX will partner with CHOC as their designated QHIO to help them meet data sharing requirements under DSA/DxF.

Founded in 1907, Hazel Hawkins Memorial Hospital is a public agency that serves as a responsive, comprehensive health care resource for its patients, physicians, and the health care consumers of the community. Hazel Hawkins Hospital is one of the few California hospitals still locally governed, and its community-based, elected board of directors protects this local status to ensure the community's health care needs are met. Through this partnership, MX will provide longitudinal health records to close gaps in care coordination while helping Hazel Hawkins Memorial Hospital meet data sharing requirements under DSA/DxF compliance.

ICYMI: "Powering Healthy Communities Through Whole Person Data Exchange" Recap

Manifest MedEx hosted half-day regional convenings on April 8 in Rocklin and on April 10 in Merced to advance opportunities to improve whole-person care coordination and address California regional priorities.

We kicked off both days with a presentation from MX's Director of Health Policy, Felix Su, who provided insight into how DxF oversight and enforcement by state agencies are poised to ramp up over the next several years, and the best opportunities to comply with and benefit from requirements for secure, real-time data sharing.

On April 8, attendees gained insight into an on-the-ground perspective on the status of CalAIM in Central and Northern CA and learned about the collaborative efforts of a health plan, provider, and community-based organization on the evolution of health data exchange in their communities.

On April 10, attendees learned the innovative ways the Fresno County Department of Public Health approaches whole person care amidst the recent launch of the Fresno County Information Exchange and heard cross-collaboration efforts between San Joaquin County leaders that support data-driven decision-making and improve population health surveillance.

Both convenings ended with transparent dialogue on real-world use cases that demonstrate how a more connected health and social care data sharing ecosystem advances innovation and equity across the local healthcare landscape.
