Evolution CA 2025: Southern Inland Recap – “Bridging Clinical, Claims, and Social Data to Accelerate Whole Community Care”

Manifest MedEx held its inaugural regional forum, Evolution CA 2025: Southern Inland, themed “Bridging Clinical, Claims, and Social Data to Accelerate Whole Community Care,” on September 9, 2025, at the March Field Air Museum in Riverside, California. 

Leaders from across the region, representing hospitals, health plans, physician groups, community based organizations (CBOs), and public health agencies, convened to participate in a full day of collaborative conversation on ways to advance health and health-related social services (HRSS) data exchange on a regional level.  

During this year’s regional forum, we focused on providing a space for connection and ideation as we all work together to advance California's priorities in whole-person care, population health, and public health. 

We kicked the day off with our keynote speaker, Elizabeth Landsberg, Director of the California Department of Health Care Access and Information, who highlighted data-driven strategies from the state level that identify and address health care transformation opportunities across California using actionable data.

Jarrod McNaughton, Chief Executive Officer, Inland Empire Health Plan, joined us for a candid conversation on the impact of Medicaid changes in the area and how fostering better health and social data exchange could improve efficiencies in a reduced reimbursement environment. 

We also had industry experts shed light on opportunities with health and social data exchange on whole person care. Dr. Vikram Kumar, M.D., MBA, FAAP, Chief Executive Officer, Ventura County Ambulatory Care, provided details on the art of the possible – motivating the audience on what is possible at the intersection of health and social services data and public health exchange. Similarly, Dr. Geoffrey Leung, M.D., Ed.M., Public Health Physician, Riverside University Health System, walked us through the Whole P.E.R.S.O.N. Health Score – a patient-centered assessment tool designed to provide a well-rounded view of patients using six domains: Physical, Emotional, Resource Utilization, Socioeconomics, Ownership and Activation, and Nutrition and Lifestyle – and shared its impressive impact on patient health outcomes.

To highlight various innovative use cases across the state, we hosted 15-minute sessions called “Spotlights: Expanding the Data Sharing Ecosystem.” Attendees learned more about ConnectIE, a social services resource directory in the Inland Empire from Rudy Valdez, Program Director, Inland Empire Health Information Organization (IEHIO), gained perspective on the importance of integrating social services organizations into health data exchange from Brit Steele, Director, Inland Housing Solutions, and heard how leveraging health data can enhance care coordination for women, infants, and children from Megan Esparza, Executive Director, California WIC Association, amongst other topics, including the importance of integrating fire and EMS in the data exchange ecosystem from Mike Ciancio, Fire Chief. Northshore Fire Protection District. 

Additional sessions included insights from MX’s Chief Information Officer, Jason Buckner, and Chief Privacy Officer and General Counsel, Elizabeth Killingsworth, who spoke on 42 CFR Part 2, the evolving landscape of health data privacy and security in California, and how Manifest MedEx is navigating through these rules and regulations. Our Senior Director, Health Informatics, Tim Polsinelli shared two examples of ways Manifest MedEx is supporting counites: 1) helping counties and managed care plans (MCPs) meet data sharing requirements of All Plan Letter (APL) 23-029 by designing a HUB model, allowing agencies and MCPs to exchange a standard set of data in a known format through MX and 2) partnering with Fresno County on a county-wide Social Health Information Exchange with two pilot use cases, including one for Youth Suicide Prevention that partners with schools.

After hosting a fun, outdoor lunch provided by food trucks from the Southern Inland area, we facilitated World Cafe Round Table discussions where attendees rotated between four topics – Assessing Social Drivers of Health with Data, Exchanging Behavioral Health and Part 2 Data, Meeting Housing Needs with Data, DxF Listening Tour: We Want to Hear From You – in which they were able to brainstorm, connect, and ask questions for 15 minutes per topic. The first three topics were led by internal MX team members, while the fourth topic was led by Kia Gaines, Healthcare Management Consultant; Michael Haynes, Healthcare Operations Senior Consultant; and Shanti Wilson, Vice President of Client Solutions from Freed Associates, a California-based healthcare consulting firm partnering with HCAI to gather feedback on the Data Exchange Framework (DxF).

We were also pleased to welcome Bill Howard, Strategic Consultant and past Board Chair of eHealth Exchange, to close out the program with a thought-provoking conversation on AI and its impact on workflows in different health industries, data interoperability, and individual health outcomes.

Lastly, before we transitioned to our afternoon cocktail reception, where attendees, speakers, colleagues, and friends could meet and network, we presented our inaugural Mosaic Award to Dolores L. Green, MBA, Executive Director, Riverside County Medical Association; CEO, Inland Empire Foundation for Medical Care; and CEO, Inland Empire Health Information Organization. This award celebrates those who bring together disparate data to create a unique, yet comprehensive picture of health data that positively impacts health outcomes.
