Meet the new CMS ADT Notification Requirements with funding from Cal-HOP

What are the new CMS requirements?

Effective May 2021, all acute care, psychiatric, and critical access hospitals are required to send event notifications to the patient’s care team when patients are admitted, discharged, or transferred to community providers. Hospitals that don’t comply risk losing their ability to receive payments from Medicare and Medicaid.

How is MX making compliance easy for hospitals?

By simply joining the MX network and setting up one data feed, hospitals can meet the new CMS requirements. Manifest MedEx will take on the heavy burden of routing ADT event notifications to the patient’s ambulatory and post-acute care team.

For a limited time, MX is offering financial incentives from the California Medi Cal HIE Onboarding Program (Cal-HOP) to help hospitals and providers participate in an HIE. Contact us today to learn about incentives for getting connected to MX’s network.

Why pick MX?

Whether your organization is managing risk, tackling leakage, increasing efficiencies in the emergency department and at discharge, creating an ACO or CIN, or reducing readmissions, you need insights and data from outside your organization.

Manifest MedEx (MX), California’s largest nonprofit health data network, is here to help you take advantage of Cal-HOP funding and give you these missing data and insights so you can improve patient care and succeed in the market. MX is the only HIE in the state leveraging claims and clinical data to offer three powerful tools: real time encounter alerts, longitudinal records and population health dashboards.

Joining MX means you’ll know when your patients are in the hospital, what prescriptions they actually filled, and their risk for a hospital readmission. You will be able to treat them quicker in the emergency department and streamline the discharge process. You will have insights into referral patterns and leakage so that you can improve patient retention.

You get all of this + funding to help you reach your goals this year.
