Manifest MedEx and SCHIO Announce Strategic Affiliation
To Connect California and Advance Statewide Health and
Social Services Data Infrastructure

We are pleased to share that Serving Communities Health Information Organization (SCHIO), one of the oldest and most advanced multi-stakeholder health information exchanges (HIEs), is now an affiliate of Manifest MedEx! Under the affiliation arrangement, Manifest MedEx will provide data sharing technology and services to SCHIO participants.
SCHIO connects more than 200 health and social services organizations, including providers, counties, skilled nursing facilities, and two local health plans, Central California Alliance for Health and San Francisco Health Plan, which together serve more than 620,000 members. Established in 1996, SCHIO was operated by Physicians Medical Group (PMG), a Santa Cruz based independent physician association, for twenty years before transitioning to operating as an HIO in 2017. Like MX, it is also designated as a California Data Exchange Framework Qualified Health Information Organization (QHIO).
SCHIO has deep roots and relationships with the communities they serve and a wealth of experience in integrating public health, social services, and community-based organizations into the data sharing ecosystem.
This significant collaboration will help communities in Santa Cruz, the Bay area, and throughout the state get critical health and social data across both networks to better coordinate whole person care and empower healthier communities.
Participants of the MX network will benefit from richer health and social data through this affiliation.

Manifest MedEx Applauds Governor Newsom for Signing SB 660 to Strengthen California’s Data Exchange Framework

Manifest MedEx celebrated Governor Gavin Newsom for signing SB 660 on October 3, 2025, a new legislation introduced by Senate Health Committee Chair, Caroline Menjivar, that reinforces accountability and participation in California’s Data Exchange Framework (DxF).
The DxF is a cornerstone of California’s vision for a “Healthy California for All,” ensuring that providers, health plans, and social service organizations can securely exchange health data to improve care, safety, and outcomes for patients. As a Qualified Health Information Organization (QHIO), Manifest MedEx serves as part of California’s digital health safety net, connecting hospitals, clinics, health plans, local health agencies, and community partners to make sure critical health information follows Californians wherever they receive care.
“Governor Newsom’s signature on SB 660 sends a strong message: data exchange is not optional, it’s essential,” said Erica Galvez, CEO of Manifest MedEx. “This law puts in place the accountability needed to ensure every provider and health plan participates fully in the Data Exchange Framework so Californians get safe, well-coordinated, whole-person care.”

Manifest MedEx’s County Population Health Reports

County health agencies rely on population health data to guide, plan, and fund initiatives that support healthier, more resilient communities, but obtaining timely and accurate health data can be challenging. This makes it difficult for health departments to have real-time actionable insights into the health of their communities, respond quickly to emerging health and social needs, and make proactive, data-informed decisions to manage population health and support whole-person community care.
Manifest MedEx is pleased to offer cost-effective County Population Health Reports that provide Limited Data Sets collected in near real-time that are aggregated for eight health domains reported across a geography (i.e., county, zip code, and census tracts). These reports include critical data and measures relevant to that facet of a population’s health and can be imported into most county databases to generate multi-dimensional dashboards and analytics.

Manifest MedEx’s Enhanced NCQA Data Aggregator Validation Services

Manifest MedEx (MX) is one of the first health information organizations in the country to earn NCQA’s Validated Data Stream designation and has obtained this designation since 2021. This designation confirms that validated data from Manifest MedEx can be used as standard supplemental data in HEDIS® reporting, eliminating the need to obtain and validate clinical data from individual providers.
As a premium service, Manifest MedEx also offers health plans “uplifted,” that is, enhanced or improved, NCQA validated data to be used as standard supplemental data for HEDIS reporting. The most common and significant challenge in using provider-sourced clinical data for HEDIS reporting is the use of non-standard codes, local codes, or non-codes by provider organizations. Improperly coded data often errors out of HEDIS engines, leading to underreporting and potentially negatively impacting health plan quality ratings, quality improvement initiatives, and reimbursement.

Introducing the CSRI Health Data Utility Capability Model
On October 28 at 10 AM PT, Civitas Networks for Health is hosting a webinar on the CSRI Health Data Utility (HDU) Capability Model, a new approach for characterizing and assessing the maturity level of HDUs across the country to support the advancement of interoperability and health data exchange to promote improvements in health and healthcare.
Developed by the Consortium for State and Regional Interoperability (CSRI), of whom Manifest MedEx is a founding member, with input from external stakeholders and Civitas, the HDU Capability Model provides a practical, stakeholder-driven approach that describes both the core capabilities of HDUs and those required to deliver value and services to the stakeholders they serve, including providers, public and private sector payers, public health agencies, researchers, and patients.

APHA 2025 Annual Meeting and Expo
Many states have developed partnerships with strong, nonprofit health data sharing organizations that have evolved beyond the clinical use cases of traditional HIEs and now serve as HDUs for their state. While they may serve a range of specific use cases and vary in terms of maturity and the breadth of network and services they provide, there are some core functions and capabilities common across all of them as they serve the real-time health data analytics needs of both the public and private sectors in their states.
Beyond supporting public health within their state, how might HDUs be leveraged to support efficient and effective public health surveillance, including syndromic surveillance, nationally, especially considering potential funding cuts to the CDC? How could HDUs collaborate to scale national public health data infrastructure in concert with the national networks to prepare for the next pandemic? What additional public health use cases could HDUs serve with their robust data sets to advance precision public health?

Manifest MedEx held its inaugural regional forum, Evolution CA 2025: Southern Inland, themed “Bridging Clinical, Claims, and Social Data to Accelerate Whole Community Care,” on September 9, 2025, at the March Field Air Museum in Riverside, California.
Leaders from across the region, representing hospitals, health plans, physician groups, community-based organizations (CBOs), and public health agencies, convened for a full day of collaborative conversation on advancing health and health-related social services (HRSS) data exchange at a regional level.
During this year’s regional forum, we focused on providing a space for connection and ideation as we all work together to advance California's priorities in whole-person care, population health, and public health.
