Fostering Collaboration and Driving Innovation
Through Regional Convenings

Manifest MedEx is committed to deepening connections within local communities and connecting with national- and state-level leaders to ensure participant needs shape policy, planning, and implementation. We are pleased to preview a few upcoming events tailored to bring stakeholders together, share knowledge, and create opportunities for meaningful engagement, with specific dates and times to follow:

  • In April, Ednann Naz, M.D., Executive Director for the Northern & Central Region, and Gabby Lopez, Executive Director for the Bay Region, will host MX participants for a day of discussion and collaboration in El Dorado Hills and Merced. These two events will give local stakeholders an opportunity to focus deeply on the health priorities specific to their communities,  identifying strategies to advance health, social, and behavioral health data sharing to address those priorities.
  • In September, Manifest MedEx will host its second annual regional forum in the South Bay Area, convening stakeholders from public health jurisdictions, community-based organizations, hospitals, health plans, and other health organizations across the region. The forum will celebrate data-driven accomplishments that have helped advance the health of Bay Area residents and examine opportunities to better integrate health, social, and behavioral health data to empower healthier communities.

MX Monthly Product Trainings

Manifest MedEx hosts monthly product trainings to help users walk through key features designed to boost efficiency and enhance productivity in MX Access, MX Notify, and our MX Admin Settings.

Manifest MedEx welcomes Anaheim Community Hospital, Calaveras County Public Health Department, PromiseCare Medical Group, Partners in Care Foundation and, as a result of our new affiliation with Serving Communities Health Information Organization (SCHIO), Central California Alliance for Health (the Alliance)Cope Health SolutionsHealth Projects CenterMerced County Behavioral Health & Recovery ServicesPajaro Valley Prevention & Student Assistance (PVPSA), and Watsonville Community Hospital, to our ever-growing network!

Founded in 2022, Anaheim Community Hospital delivers patient-centered care rooted in empathy, clinical excellence, and integrity—ensuring every person they serve receives the attention and dignity they deserve. This 98-bed general acute hospital aims to be a trusted healthcare partner for its community—one that consistently provides safe, coordinated, and compassionate care. As the largest nonprofit qualified health information organization (QHIO), Manifest MedEx is pleased to help Anaheim Community Hospital achieve DSA compliance through near real-time data sharing.

The Calaveras County Public Health Department (CCPH) aims to improve the health and well-being of everyone in Calaveras County. Founded in 1946 with the formation of the Mark Twain Hospital District, CCPH focuses on improving its department's operations, tracking diseases and sharing information, making it easier for its community to access health services, and working more closely with community groups. While valuing community engagement, social responsibility, excellence and quality improvement, and health equity, CCPH offers a range of units, including, but not limited to, Family Health, Epidemiology and Informatics, Infectious Disease Control and Prevention, and Health Response and Preparedness. Manifest MedEx is thrilled to provide real-time admission, discharge, and transfer (ADT) alerts, longitudinal patient records, and a reporting infrastructure to help CCPH close health, social, and behavioral health data-sharing gaps in public health while also supporting public health planning and response.

Founded in 1996, the Central California Alliance for Health (the Alliance) is a local public agency that serves members in Mariposa, Merced, Monterey, San Benito, and Santa Cruz counties. The Alliance partners with contracted providers to promote prevention, early detection, and effective treatment, and improve access to quality health care for those they serve. Their work results in the delivery of innovative community-based health care services, better medical outcomes, and cost savings. The Alliance aims to provide accessible, quality health care guided by local innovation. Manifest MedEx will provide real-time data to help the Alliance improve whole person care across its network.

PromiseCare Medical Group has the longest-serving, largest Independent Physician Association network in the Inland Empire. This network strives for excellence in everything it does, from providing quality care to the most chronically ill and frail to meeting the changing healthcare needs of our surrounding communities with a forward-thinking perspective. PromiseCare Medical Group places the needs of its patients first, while also focusing on excellent clinical outcomes, patient safety, and exceptional service. Manifest MedEX looks forward to partnering with PromiseCare to manage population health and improve care coordination.

Partners in Care Foundation is a recognized leader in care coordination and community-based health services, aiming to improve health outcomes by bridging healthcare and community services — empowering people to live safely, heal fully, and thrive. Since 1997, it has worked to bridge the gap between medical care and community-based services, the non-medical support people need to live safely, independently, and with dignity in their own homes. Through this partnership, Manifest MedEx will provide real-time health and health-related social services (HRSS) data to better engage high-risk, high-acuity members enrolled in its programs.

COPE Health Solutions (CHS) is a national tech-enabled services firm powering success for health plans and for providers in risk arrangements. Its comprehensive, NCQA-certified population health management platform and highly experienced team deliver deep expertise, proven tools, and processes to improve financial performance and quality outcomes for all types of payers and providers. Founded in 2002, CHS aims to accelerate sustainable population health management and drive value-based care transformation. Manifest MedEx is delighted to provide health, social, and behavioral health data through the delivery of ADTs and CCDAs to advance care coordination for CHS' Enhanced Care Management (ECM) population.

Health Projects Center is a 501(c) (3) nonprofit corporation that has supported Californians in living and aging safely at home since 1980. Serving  Monterey, San Benito, Santa Cruz, San Luis Obispo, and Merced Counties, Health Projects Center empowers individuals and families to navigate the challenges of aging with dignity and confidence. Manifest MedEx looks forward to supporting the Health Projects Center as it continues to deliver high-quality services and programs focused on care management, caregiver support, and workforce development. Through this partnership, MX will provide real-time ADTs via MX Notify and clinical and discharge summaries via MX Access, helping ensure timely access to critical information. Together, these tools will strengthen care coordination and better support the Health Projects Center’s patient population.

The Merced County Behavioral Health & Recovery Services (BHRS) is committed to empowering their diverse community with hope, recovery, and wellness by providing comprehensive, holistic care. BHRS provides comprehensive mental health and substance use disorder services to county residents, supporting access, quality improvement, and community well-being through programs such as the Behavioral Health Quality Improvement Program (BHQIP). Manifest MedEx will partner with BHRS to improve data-driven oversight of behavioral health populations, strengthen quality improvement and outreach capabilities, and reduce data silos across the healthcare ecosystem. MX will serve as the County’s DxF infrastructure, aggregate and standardize data from hospitals, plans, and providers, and deliver alerts, reports, and analytics aligned with county priorities.

Founded in 1991, Pajaro Valley Prevention & Student Assistance (PVPSA) aims to improve the quality of life for children, youth, and families in the greater Pajaro Valley by providing health education, mental health, and substance use services, and by advocating for public policies that protect the well-being of the community. PVPSA provides comprehensive health education, mental health services, substance abuse and intervention and prevention services, and direct counseling to the students and families of the Pajaro Valley, including the Pajaro Valley Unified School District (PVUSD), the largest school district in Santa Cruz County, which also includes North Monterey County. Manifest MedEx will deliver real-time ADT notifications to support PVPSA in providing services to its ECM members.

As a 106-bed general acute care facility, Watsonville Community Hospital offers a comprehensive range of medical and surgical services to the culturally diverse Pajaro Valley and California’s Central Coast. Founded in 1895, Watsonville Community Hospital aims to be the trusted, equitable healthcare partner and provider that its diverse families, friends, and neighbors deserve. Manifest MedEx will help Watsonville reduce gaps in health, social, and behavioral health data sharing with other healthcare partners in Santa Cruz and surrounding communities.
