2025: A Year of Evolution, Expansion, and Partnership
for Manifest MedEx and California
2025 marked a year of evolution for Manifest MedEx that has advanced a more connected, data-driven—and efficient—future for California’s health system. As we strengthened collaborations, broadened our reach, integrated hard-to-reach data, and advanced the tools that power whole-person care, we continued to build momentum toward a healthier, more equitable state.
Among the highlights this year, MX worked to:
Power Progress By Integrating Secure Health, Behavioral Health, and Social Data
- Manifest MedEx began connecting health-related social services organizations (HRSS) with providers and health plans through our nonprofit health data network, enabling the secure exchange of both health and social services information in support of CalAIM. By joining our network, community-based organizations receive real-time ADT notifications, enabling care teams to quickly coordinate support, such as housing, transportation, and medically tailored meals, to better meet individuals’ needs.
- We announced collaborations with three mission-driven organizations to advance the secure, seamless sharing of behavioral health and social data to accelerate whole-person data sharing and enhance care coordination. MX now partners with:
- NinePatch® — To launch a statewide social health information exchange (SHIE) platform that allows users to document and view social drivers of health (SDOH) information and leverage tools, such as needs assessments, program enrollment, closed-loop referrals, and consent management, alongside health information from the more than 50 million longitudinal health records on MX’s health data network based on roles and permissions.
- Findhelp — To make social care referral information visible alongside health information to advance whole person care and care coordination across disparate teams and organizations.
- California Mental Health Services Authority (CalMHSA) — To support secure and appropriate ADT data exchange between behavioral health providers and broader care teams across California.
- In alignment with AB 352, we enhanced our product suite to comply and to continue protecting the disclosure of sensitive health information, including data related to reproductive health and gender-affirming care.
- Manifest MedEx successfully launched a significant upgrade to our real-time ADT notification platform, MX Notify, delivering a faster, more intuitive experience for our portal users. The enhanced features streamline workflows and improve care coordination across teams, freeing up valuable time for care teams to focus on what matters most—supporting their patients and members.
- Manifest MedEx expanded population health management services to offer cost-effective County Population Health Reports that provide Limited Data Sets collected in near real-time and aggregated across eight health domains for a geography (i.e., county, zip code, and census tracts). These reports include critical data and measures relevant to that facet of a population’s health and can be imported into most county databases to generate multi-dimensional dashboards and analytics.
- To help health plans improve data quality, Manifest MedEx also began offering enhanced data services, such as for our NCQA Data Aggregator Validation Services, that map local codes to standard codes across 10 data domains, several of which are relevant to HEDIS reporting. Standardizing thousands of data elements across these data domains to the correct codes ensures consistency across multiple sources, helps identify and correct errors, and ensures the data is valid for reporting.
- Serving Communities Health Information Organization (SCHIO), one of the oldest and most advanced multi-stakeholder health information exchanges (HIEs), is now an affiliate of Manifest MedEx! This significant collaboration is helping communities in Santa Cruz, the Bay Area, and throughout the state get critical health and social data across both networks to better coordinate whole-person care and empower healthier communities. Manifest MedEx is working with partners at the local, state, and national levels to support secure, appropriate data sharing across communities and public health initiatives.
- This year, we welcomed four new Regional Executive Directors — Eva Williams, Dr. Ednann Naz, Gabby Lopez, and Charles Alm — strengthening our commitment to supporting the health and well-being of communities across California at a local, on-the-ground level. Their leadership expanded our regional presence and deepened relationships with participants dedicated to improving community health. Building on this momentum, we look forward to creating opportunities for collaboration and partnership to elevate health and wellness in the years ahead.
- In September 2025, Manifest MedEx hosted its first regional forum, Evolution CA 2025: Southern Inland, bringing together industry leaders and participants to explore how exchanging clinical, claims, and social services data can accelerate whole-person care. Attendees heard from state and local health leaders, learned about tools such as the Whole P.E.R.S.O.N. Health Score, and reviewed real-world use cases — including housing support and coordinated care for populations who need more support. The forum also featured collaborative “World Café” sessions on social drivers of health, behavioral health, housing needs, and the CA Data Exchange Framework, and concluded by honoring a key community leader and MX board Member, Dolores L. Green, with the inaugural MX Mosaic Award.
Expand Secure, Seamless Data Exchange Across CA
- Thanks to our participants, public health officials, and health policy leaders who make health data exchange possible, today, we securely share more than 50 million individual records and deliver on average 3 million ADT notifications per month across a network of more than 2,700 healthcare providers, 150+ hospitals, 12 county health agencies, and 19 health plans.
- Manifest MedEx is delighted to welcome new participants to our network, including the California Mental Health Services Authority, County of San Luis Obispo's Public Health Department, El Dorado County’s Behavioral Health and Public Health divisions, Heritage Health Network, Illumination Foundation, Santa Clara Family Health Plan, and more!
- We successfully met the requirements of HITRUST CSF® certification for the seventh consecutive year. This demonstrates MX’s compliance with the highest levels of security in the healthcare industry and places our nonprofit among the elite few QHIOs that have continuously maintained this certification.
Manifest MedEx Partners with NinePatch® to Launch Statewide Social Health Information Exchange for Whole Person Care in California

Manifest MedEx is pleased to announce a new partnership with NinePatch®, a leading SaaS platform for Whole Person Health and Referral Information Exchange, to launch a statewide social health information exchange (SHIE) platform that will display social drivers of health (SDOH) information and provide tools, such as care assessments, program enrollment, closed-loop referrals, and consent management, with the more than 50 million longitudinal health records on MX’s health data network. MX’s new SHIE platform will be powered by NinePatch technology and made available to MX’s network of more than 140 hospitals, 19 health plans, 12 county health agencies, and 2,700 providers across the state.
Providers and health organizations, including community-based organizations (CBOs), that participate in the MX network will be able to appropriately and securely share an individual’s health history, social needs, and social services information, as permitted. MX’s integrated tools combine this information into one unified view, helping healthcare providers, social service providers, and Medi-Cal plans deliver Enhanced Care Management and Community Supports services under CalAIM more effectively.

Manifest MedEx will begin offering its SHIE platform to California county health agencies in 2026. In addition to its partnership with NinePatch for its SHIE, MX is also collaborating with Findhelp, a social care referral platform, to support seamless, secure, and appropriate access to SDOH data for participants in the MX network (more below).

Manifest MedEx and California Mental Health Services Authority (CalMHSA) Partner to Provide Free Hospitalization Notices to California Counties

“With this partnership, multi-disciplinary care teams focused on whole person care will have a clearer and more timely sense of what the individuals in their care need,” said Erica Galvez, CEO, Manifest MedEx. “Care coordination only works when everyone involved has accurate information, when they need it. Behavioral health has been left out of data exchange for far too long, creating gaps in that coordination, and this collaboration helps close those gaps.”
The California Data Exchange Framework (DxF) requires hospitals and emergency departments, including acute psychiatric hospitals, to facilitate ADT exchange, serving as an accessible first step towards improving behavioral health care coordination. As part of the state’s Medi-Cal transformation under CalAIM, providers and health plans are expected to receive and act on these real-time ADT notifications to support timely follow-up care and reduce readmissions.

Manifest MedEx Now Offering Single-Sign On (SSO)
Through this new initiative, MX will provide CalMHSA with admit, discharge, and transfer (ADT) notifications for inpatient and emergency department encounters involving individuals overseen or treated by participating county behavioral health organizations. CalMHSA will also share notifications back with the MX network. Through this two-way exchange, both organizations help care teams gain a more comprehensive and timely understanding of individuals, ultimately improving care coordination for follow-up needs.

New Health-Related Social Services (HRSS) Role in MX Access

Manifest MedEx now enables health-related social service (HRSS) organizations to gain additional insight from longitudinal health records in MX Access. MX Access provides social service teams with a limited view of the minimum necessary clinical data to support their care coordination activities and community support services under CalAIM, such as medications, lab results, and diagnoses, enabling more informed, coordinated, and effective care.

MX Welcomes New Participants
Manifest MedEx welcomes Linea Care and, as a result of our new affiliation agreement with Serving Communities Health Information Organization (SCHIO), Manning Gardens Care Center, Inc., and Salinas Valley Health to our ever-growing network!
An AI-driven platform founded in 2024, Linea Care empowers heart failure patients with personalized, proactive care, elevating their quality of life and helping them live with greater confidence. Linea’s platform combines generative AI, evidence-based protocols, and a dedicated care team to prevent re-admissions for heart failure patients. MX is delighted to provide real-time ADT notifications for their patients, along with longitudinal health records to help them make better informed decisions.
Manning Gardens Care Center, Inc. was founded in 1962, and they specialize in providing a variety of rehabilitation and skilled nursing services to help our residents recover from surgery, injury or serious illness or to assist them as they go through their end-of-life stages. Their goal is to enrich the lives of our residents with quality care, compassion, and exceed their expectations. With 59 Medicare and Medi-Cal certified beds and contracts with the VA and various insurance and hospice companies, Manning Gardens Care Centers, Inc. strives to meet residents' needs by providing them with quality of life and dignity, as they struggle with various health challenges. By partnering with MX to access health and social services data, Manning Gardens Care Center will improve care coordination.
Serving Monterey County since 1953, Salinas Valley Health delivers exceptional, compassionate, and culturally sensitive care, holding true to their mission of improving the health and well-being of their community. At the core of the system is a 269-bed acute care hospital with specialized facilities and programs, such as a Regional Heart Program and Comprehensive Cancer Center, an Emergency Department, and a Level III Neonatal Intensive Care Unit. MX looks forward to providing comprehensive longitudinal health records and real-time health information to help Salinas Valley Health maintain quality care in the communities it serves.

New Updates to MX Participant Policies
Over the last several years, MX Participants have expressed a need for the ability to exchange a wider variety of data to better enable additional health and social supports within California. In an effort to provide these enhanced capabilities for our Participants, MX is updating its Participant Policies to permit the optional exchange of certain sensitive data, including substance use disorder (SUD) data, provide a pathway for the exchange of additional social data, and establish an approved use case for coroners and medical examiners.
We believe that these changes allow MX to provide more and better information to all our Participants, resulting in better care, lower costs, and more efficient and robust exchange.
At a high level these policy changes address:
- Optional Sensitive Health Information Exchange. New Policy PP-26 “SUD Data and Certain Behavioral Health Data” has been added to permit Participants to opt-in to the exchange of certain sensitive data, including data protected by 42 CFR Part 2 and related state laws.
- Additional Requirements for Reproductive Health Data. Policy PP-8 “Information Subject to Special Protection” has been updated to include additional requirements related to accessing Patient Data related to gender affirming care, abortion and abortion-related services, and contraception to clearly align with the requirements of AB352.
- Community Based Organizations (CBOs). Policy PP-25 “Disclosures Pursuant to Patient Consent/Authorizations” has been updated to provide a more detailed description of access permissions and restrictions for CBOs seeking to submit or retrieve Patient Data pursuant to an individual consent/authorization.
- Coroner & Medical Examiner Use Case. Policy PP-4 “Permitted Uses of Data and the System” has been updated to permit coroners and medical examiners to access Patient Data as permitted and limited by HIPAA.
- Clarification and Clean Up. Minor edits have been included throughout to address areas where prior language either: (1) frequently resulted in questions from our Participants or (2) was no longer relevant.
These policy changes align MX policy with evolving state and federal data sharing requirements, increase transparency and clarity on use cases supported by MX, and expand the types of Patient Data that may be exchanged with MX. Further, they do not impose any new required action on the part of our Participants to comply unless and until a Participant chooses to opt-in to the optional sensitive health information exchange.
